NRHS Coordinated Care Plan
NHealth offers a unique health plan called the Coordinated Care Plan. It is currently available to Norman Regional Health System employees. This plan brings together all the resources of the NHealth to help patients or members manage their health conditions. Members may even qualify for financial incentives through this plan.
Some of the benefits of the coordinated care plan are:
- A member receives health education, health coaching and self-management support
- Collaboration with a provider to strengthen the patient-provider relationship
- Case management services to assist in the continuum of care
- Financial incentives
- Qualifying members may be eligible for reimbursement on the costs of co-payments and pharmacy expenses associated with certain chronic conditions
