Coordinated Care Plan


qualificationsThe Coordinated Care Plan is available to all Norman Regional Health System employees as a benefit plan. Members must have a Primary Care Provider or PCP that is an NHealth network provider. You can click here to see a list of our providers. Members with 1 or more of the following 5 chronic conditions may enroll into the Chronic Disease Management option of the Coordinated Care Plan.

  • Asthma
  • Cardiovascular Disease (CVD)
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Congestive Heart Failure (CHF)
  • Diabetes

Requirements for Participation in the Chronic Disease Management Program

Members must first complete the enrollment process in the Care Coordination Plan. Then he or she attends provider visits as scheduled in a treatment plan. The member also completes physician ordered lab studies per the treatment plan. Medication compliance is also an important part of the plan. This plan also requires active communications with Care Coordination staff at least two times a year.
